Formalize backlog -> approved -> ready lifecycle and update cockpit lanes
Codify the canonical operator lifecycle as backlog -> approved -> ready -> running -> review -> done. Currently the backlog and approved lanes exist as informal staging/triage lanes without a scripted promotion path between them. This item formalizes the full lifecycle: adds a factory-promote-backlog-to-approved.ps1 script, adds a factory-promote-approved-to-ready.ps1 script (or extends existing), updates the lifecycle contract guard, updates QUEUE_FLOW.md and BUILD_FACTORY_SYSTEM_CONTEXT.md, and ensures the operator cockpit UI reflects all six lanes in the correct order with correct counts and actions.
