Measure and improve PowerShell test coverage and quality
After the Pester 5 migration (BF-195–200), the 108 migrated tests reflect historical coverage but have not been audited for completeness or quality. This item establishes coverage measurement for the factory PowerShell scripts, identifies critical untested paths, and adds missing tests. Goals: (1) measure current coverage baseline using Pester code coverage or equivalent, (2) identify high-risk untested scripts and functions, (3) add missing tests to reach meaningful coverage targets, (4) establish a coverage gate in factory-run-required-validation.ps1 so coverage cannot regress. Adds an enforced non-regression coverage gate to prevent coverage backsliding.
