Exchange lane model + state machine — transition rules, guards, and audit trail for all 9 exchange states
Implement the canonical exchange state machine service (lib/services/exchange-state-machine.ts). Define legal transitions between the 9 exchange lanes: candidate→triage, triage→approved|rejected|blocked, approved→packaged, packaged→applied, applied→verified|rejected, any→blocked, any→superseded. Implement transition guards (e.g. packaged requires artifact present, applied requires target repo confirmation). Each transition must write an audit_event record (from BF-213 schema). Expose a transition() method that validates guard, writes audit, and updates exchange record state atomically. Reject illegal transitions with a typed ExchangeTransitionError. Write tests covering all legal transitions and all illegal transition attempts.
