Queue drift monitoring and self-healing script hardening
Add factory-monitor-queue-drift.ps1 to detect and auto-repair queue lane drift: duplicate IDs across lanes, lane status/state mismatches, and .changelog/next.md workspace noise. Wire the monitor into factory-execute-work-item.ps1, factory-open-pr.ps1, factory-prepare-pr.ps1, and factory-detect-merged-prs.ps1 so drift is caught and corrected autonomously at every lane-touching operation. Also harden generate-changelog-entry.ps1 with idempotency guards to prevent repeated appends.
